In-reach Rehabilitation Team are looking for a Social Worker – Senior to join their Princess Alexandra Hospital team. The purpose of this role is to deliver senior Social Work services within the multidisciplinary In-reach Rehabilitation team, working within a shared care model to provide specialist rehabilitation services.

About the Role Apply increasingly independent clinical judgement to Social Work, in areas such as psychosocial and risk assessments, discharge planning and case management to support complex decision making with only occasional clinical/professional supervision.Adapt routine clinical practice as required based on clinical evidence with minimal supervision and a high degree of independence.Assist in the identification, development, implementation and/or review of clinical practices, procedures and protocols in work areas using the principles of evidence based practice under the guidance of senior staff.Find out about the role in more detail and how to apply in the attached Role Description.
