The Wide Bay Hospital and Health Service is seeking a skilled and compassionate Senior Social Worker to join our Geriatric Evaluation Management / Rehabilitation Hospital in the Home (GEM/GEMR HITH) service.
This innovative program focuses on improving the health outcomes of older patients in their own homes, reducing unnecessary hospital admissions, and supporting a quicker recovery through tailored care.

The position will provide leadership in the coordination and delivery of evidence-based Social Work services across the care continuum to ensure optimal assessment, clinical intervention, and discharge planning occurs for health service consumers as part of a broader multidisciplinary team.Work collaboratively with consumers and multidisciplinary professionals to provide high level clinical care and a therapeutic environment conducive to the dignity and wellbeing of consumers and carers within Mental Health and Specialised Services.
Provide high level profession specific services, consultation and support, as required, within the team and service.The Geriatric Evaluation Management / Rehabilitation Hospital In The Home (GEM/GEMR HITH) is an initiative of the Wide Bay Hospital and Health Service funded through the Queensland Health Improvement Unit.
The service is focused on improving health outcome for patients over the age of 65 years or 50 years for Aboriginal and Torres Straight Islander people, supporting avoidable admission and reducing length of stay in hospital for patients with age related physical, functional and /or cognitive decline associated with frailty.Find out more in the Role Description.Our offer:You will receive a competitive remuneration package value up to $76,387 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Permanent Part Time, 38 hours p.f.)
