The Wide Bay Hospital and Health Service is seeking a skilled and compassionate Senior Social Worker to join our Geriatric Evaluation Management / Rehabilitation Hospital in the Home (GEM/GEMR HITH) service.
This innovative program focuses on improving the health outcomes of older patients in their own homes, reducing unnecessary hospital admissions, and supporting a quicker recovery through tailored care.Key Responsibilities:Deliver high-level clinical Social Work services and professional expertise to patients/stakeholders of the GEM/GEMR HITH service.Provide leadership in the coordination and delivery of evidence-based Social Work services across the care continuum.Work collaboratively with consumers and multidisciplinary professionals to provide high-level clinical care.Ensure optimal assessment, clinical intervention, and discharge planning occurs for health service consumers.Our Offer:You will receive a competitive remuneration package value up to $76,387 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Permanent Part Time, 38 hours p.f.
