Located in Rushworth, Waranga Health is a 36-bed aged-care facility.
The health service includes a four-bed acute care unit, a district nursing service, outpatient ambulatory consulting services, and a dedicated palliative care suite and respite bed.
Care and treatment are provided 24 hours a day by a talented multidisciplinary team working across a range of medical, nursing, and allied health services.
About GV Health - Tatura and Parkvilla Campus Tatura Hospital and Parkvilla provide Sub-Acute and Aged Care Services.
The 8-bed hospital provides Sub-acute hospital care, specializing in palliative care and voluntary assisted dying services.
Parkvilla is a Residential Aged Care facility for 13 residents and two transitional care clients, a 'home away from home.'
Our campus prides itself on the delivery of excellent care.
We currently hold a 4 Star rating in Residential Aged Care, NDIS Practice Standards, and National Safety and Quality Health Service Standards compliance.
About the Role This position is responsible for the provision of a high-quality Social Work service across the acute wards and residential aged care facilities at the Tatura and Rushworth campuses.
This role is well suited to an experienced Social Worker who enjoys working both independently and as part of a multidisciplinary team.
You will be well supported as you manage a diverse caseload providing practical and emotional support to patients and their families to assist in the management of changes to their health and wellbeing.
The Social Worker will be based at the Rushworth and/or Tatura Campus and be well supported by the Social Work Department based at GV Health Shepparton.
We are looking for: An approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)Demonstrated skills in assessment and intervention options for patients with diverse and complex psychosocial circumstancesDemonstrated ability to work effectively both independently and as an active member of an interdisciplinary team inclusive of medical, nursing, allied health, and managementDemonstrated time management and organisational skills in the management of a clinical caseload and clinical responsibilitiesPlease attach a copy of your qualification in support of your application.
