Role requirement   The successful applicant must hold a tertiary degree (or equivalent) qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW). This position requires the incumbent to operate a class C motor vehicle and an appropriate licence endorsement to operate this vehicle is required. Proof of this endorsement must be provided prior to the commencement of duty. Aged Care Pre-Employment Screening: Applicants are advised this position works in an identified Aged Care Facility as per the Aged Care Act 1997 and therefore requires employees and volunteers to have a current National Police Certificate.  Queensland Health will facilitate applicants obtaining the above National Police Certificate.   The ideal applicant will be someone who has proven ability and can demonstrate the following: Demonstrated clinical knowledge and skill in providing clinical social work services with community based clients who present with chronic and aging health conditions and/or rehabilitation focused needs. Demonstrated ability to effectively communicate and work cooperatively within a multidisciplinary team. Demonstrated understanding of, and ability to apply person centred practice principles, with an understanding and valuing of individualised goal planning and self-management approaches. Ability to work flexibly and responsively, in a fast paced and complex health environment, with demonstrated problem solving skills and organisational ability. Demonstrated commitment to ongoing learning and ability to contribute to team quality activities. Generous remuneration package value up to $102,964 p.a.comprising: Salary rates between $38.70 - $56.89 p.h.
