Role requirement The successful applicant must hold a tertiary degree (or equivalent) qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW). This position requires the incumbent to operate a class C motor vehicle and an appropriate licence endorsement to operate this vehicle is required. Proof of this endorsement must be provided prior to the commencement of duty. Aged Care Pre-Employment Screening: Applicants are advised this position works in an identified Aged Care Facility as per the Aged Care Act 1997 and therefore requires employees and volunteers to have a current National Police Certificate. Queensland Health will facilitate applicants obtaining the above National Police Certificate. The ideal applicant will be someone who has proven ability and can demonstrate the following: Demonstrated clinical knowledge and skill in providing clinical social work services with community based clients who present with chronic and aging health conditions and/or rehabilitation focused needs. Demonstrated ability to effectively communicate and work cooperatively within a multidisciplinary team. Demonstrated understanding of, and ability to apply person centred practice principles, with an understanding and valuing of individualised goal planning and self-management approaches. Ability to work flexibly and responsively, in a fast paced and complex health environment, with demonstrated problem solving skills and organisational ability. Demonstrated commitment to ongoing learning and ability to contribute to team quality activities.
