Are you an experienced Social Worker with a passion for Neurodegenerative Diseases? About the Opportunity Our Community Social Work team is searching for a candidate with end of life, palliative care and/or neurodegenerative disease experience.
You will work closely with families and carers providing home visits and some clinic work.
You will work within a multi-disciplinary team and in conjunction with other services such as NDIS providers.
In this role, community experience is desirable and a passion for Neurodegenerative Diseases such as Motor Neurone Disease is a MUST!
You will be working with clients from point of diagnosis to end of life.

What You'll be Doing
This position is responsible for providing specialist Social Work service to consumers and their carers particularly in the Neurodegenerative Disease Supportive Care Service to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
The position will also participate in professional development, clinical supervision, teaching, quality activities, and other relevant departmental and administrative duties.
Minimum Requirements
To be considered for this position, please ensure you address the below questions as thoroughly as possible:
A Social Work degree and eligibility for the Australian Association of Social Workers (AASW). Extensive post graduate experience working as a Social Worker in relevant clinical areas in a health setting, including a demonstrated understanding of complex health conditions and the impact on wellbeing. Demonstrated experience in prioritising work and managing stressful situations; and applying advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature. Demonstrated ability to lead complex casework including communication and collaboration with the patient and other stakeholders in order to maximize positive patient outcomes and collaborative partnerships. Demonstrated counselling skills to address client distress, and ability to provide counselling/therapies that focus on the psychological, behavioural, relational or social causes of distress. Experience working independently and collaboratively within a multi-disciplinary team. Demonstrated high level communication skills (written and verbal), computer and prioritisation skills. Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and the clinical specialty. Applicants must possess an Australian C-Class driver's license (including P1 or P2).
