What you'll be doing
The Sustaining NSW Families (SNF) program is a Nurse-led, structured sustained health home visiting program for children and families from pregnancy to 2 years of age.

The SNF Social Worker will provide specialist social work and counselling interventions as part of the SNF multidisciplinary team with an early intervention focus that aims to enhance parenting capacity, promote healthy family functioning, support families at a time of change and promote infant mental health. 



Selection Criteria Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW).Extensive experience, with specialism working with families with complex clinical presentations in relevant clinical settings such as perinatal mental health, child protection, domestic violence, and substance misuse and able to form therapeutic relationships built on healthy boundaries.Demonstrated advanced specialist therapeutic skills in conducting complex psychosocial assessments and counselling with the ability to respond to the needs of vulnerable families in stressful situationsDemonstrated ability to utilise problem solving skills and a multidisciplinary team approach in planning, delivery, and coordination of patient care.Extensive expert clinical experience providing clinical consultation and facilitating case reviews, in the contact of family centred casework within a community based multidisciplinary teamExtensive demonstrated experience in developing and maintaining cooperative professional relationships, with other health teams, government, and NGO agencies through the use of well-developed interpersonal and conflict resolution skills.Demonstrated high-level verbal and written communication skills with high level time management,Evidence of current unrestricted driver's licence and the ability to maintain. Ability and willingness to travel as the role requires. This may involve driving long distances and overnight stays.
