The Social Worker Level 3 will act as a Care Coordinator to ensure provision of high quality social work services and care coordination to the clients of the service.The successful incumbent will utilise advanced professional judgement in providing psychosocial assessments, interventions, and support for people with chronic health conditions, complex social needs, and those at risk of hospitalisation.
