Together we can do more. ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low-intensity therapy in their own homes as well as assisting with the decision-making process around long-term care service supports.
As the Social Worker in ITACS, you will provide social work and case management responsibility for a number of cases from admission to discharge.
You will provide early psycho-social assessments, care planning, education advocacy for the needs of clients, families and/or carers, liaise with community agencies, the clients' General Practitioner, counselling and carer support.
The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low-intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support.
