Employment Status: Temporary Part Time (40 hours per fortnight) Salary Classification: Social Worker Level 1 Year 2-4 or Level 2 Year 1-4.
Remuneration: Social Worker Level 1 Year 2 – 4: $37.06 - $41.80 per hour + superannuation + annual leave loading + salary packaging options.
Remuneration: Social Worker Level 2 Year 1 - 4: $44.57 - $51.04 per hour + superannuation + annual leave loading + salary packaging options.
THE ROLE: Calvary Mater Newcastle is currently seeking a Social Worker Level 1-2.
This position will be primarily based in the general medicine, surgical and coronary care team, however there are opportunities to utilise your skills and knowledge as part of a supportive department across all areas of practice within CMN.
The successful applicant is responsible for the provision of social work services and the maintenance of professional social work standards.
This responsibility will include the provision of social work services with patients, their families/ carers, liaison with other health care professionals within the hospital and external agencies, participation in MDT, and active contribution to the social work department.
This position requires the successful applicant to work Thursday/ Friday, with other day to be negotiated.
This position is a fixed term contract until November 2025.
DUTIES: Conduct comprehensive psychosocial assessments of patients attending the hospital.
Provide support, counselling and other evidence-based intervention strategies to enhance social functioning.
Facilitate discharge planning in consultation with multidisciplinary team.
Maintaining a high standard of written communication, with accurate and timely medical/clinical record entries.
ABOUT YOU: Demonstrated effective interpersonal and verbal and written communication skills in building and maintaining professional relationships.
Demonstrated ability to work effectively as a member of a multidisciplinary team.
Demonstrated ability to liaise with and maintain effective links with relevant external organisation.
Demonstrated knowledge and skills in working with patients and their carers with life-limiting and / or terminal illnesses, including end of life.
WHAT YOU BRING: Demonstrated ability to effectively prioritise and organise own work / caseload.
Ability to liaise with and maintain effective links with relevant external organisations.
Commitment to evidence-based practice and ongoing personal and professional development.
Bachelor of Social Work or other equivalent qualification and eligible for membership of the AASW.
