The social work department provides personal counselling, mediation and therapeutic interventions supporting patients and their families to adjust to diagnosis, trauma and emotional responses to illness and treatment.
Flexibility to work across a Monday to Friday roster is essential to this role.  About the Role  An exciting opportunity exists for a highly motivated individual to join our Hospital Discharge Planning Team. If you are interested, we would love to hear from you.
Duties Provide a high quality, comprehensive service to inpatients of the hospital to facilitate timely and effective discharge planning.Conduct thorough initial assessment – establish previous and current level of function, supports and services.Work collaboratively in a multidisciplinary team to deliver quality patient outcomes.Implement and coordinate a timely plan for discharge, liaise with community service providers and organise home support where relevant and required.Provide information, education and support to patients and families.Your Skills and Experience Current registration with AHPRA as registered nurse.Minimum of 3 years' experience in an acute health care setting.Ability to work autonomously and also as a team player.Ability to problem solve and negotiate with patients and carers to achieve optimal outcomes.Adequate clinical knowledge to understand complex patient cases.Sound knowledge of My Aged Care system and ACAT referral criteria.What's in it for you?
