The Listowel Wingham Hospitals Alliance was formed on July 1, 2003 as a partnership between Listowel Memorial Hospital and Wingham and District Hospital. The Hospital has an active staff of 10 Family Practitioners as well as several consultant staff.Hospital services include: Emergency, Operating Rooms, Diagnostic and Ambulatory Care Units (December 2007)Maternal/Newborn UnitDiabetes/Nutrition CounselingSpeech-LanguagePhysiotherapyBreast Health CentreFully integrated electronic patient health record with Family Health Team and Physician offices.The Wingham and District Hospital is a modern, progressive acute care facility providing medical, surgical, obstetrical, paediatric, chronic and rehabilitation bed and other services, including 24-hour emergency coverage.Hospital services include: Emergency department with 24-hour on-call coverageRehabilitation department including physiotherapy, occupational therapy and speech language pathologyMultiple outpatient specialist clinics held on a regular basis to provide local serviceAffiliation with London Regional Cancer Centre enabling chemotherapy treatment to be provided locallyMany community health related education programsFully integrated electronic patient health record with Family Health Team and Physician offices.Position: Social Worker Region: Huron and Perth Description: Responsible for providing direct clinical services to patients and their families within the assessment and treatment framework of social work knowledge and skills.This is a part-time position that is anticipated to begin as soon as possible. The successful candidate will be 22.5 hours per week. Hours are subject to change.Responsibilities: To provide consultation to physicians, nurses and other relevant disciplines concerning the social and emotional factors which bear upon the patient's treatment.To maintain close communication with the physician and other professionals directly involved with the patient's treatment.To assess the psychosocial needs of patients at this hospital and aid patients in connecting with programs in the community to meet these needs.To act as an advocate on behalf of patients and their families, both in hospital and in the community.To plan and coordinate, where appropriate, programs in areas of social work expertise.To maintain current and proper records necessary for effective patient treatment and collaboration.To participate in staff meetings, hospital committee meetings, professional conferences and educational workshops.Lead, support or participate in key activities or action plans as identified on applicable Operational Plans.Active member of applicable team/department meetings, huddles, and patient care rounds.Assists in the coordination of patient discharge by collaborating with the patient, family, and multidisciplinary team.Other duties as assigned.Qualifications: Master's Degree in Social Work from an accredited School of Social Work.Member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW).Minimum 3 years previous related experience preferred.A knowledge and appreciation of diagnostic and treatment procedures utilized by related disciplines, and an ability to work constructively with other health professionals an asset.Minimal degree of physical effort.High degree of mental effort and analysis.Salary: $45.17 - $56.45 (5 step grid based on experience) Terms of Employment: Part-time Deadline for Application: Friday, October 18, 2024 How to Apply: Interested applicants are asked to apply, in writing, complete with full resume, indicating their qualifications and pertinent skills by email to:
