Social Worker HP3/HP4 - Sunnybank Private Hospital Permanent Part Time Provide supportive and coordinated guidance, both clinically and administratively to patients across Inpatients and Outpatient Day Therapy Programs.
We are looking for an experienced hospital Social Worker with a passion for excellence in customer service delivery to join our team.
Needing availability 3 days per week: Monday, Wednesday, Fridays.

Selection Criteria: Appropriate tertiary qualifications in Social Work.Eligibility for membership of the Australian Association of Social Workers.Ability to work autonomously (sole therapist).Excellent organisation, time management and self-management skills.Highly developed communication skills and interpersonal skills.Ability to work within and provide leadership within a multidisciplinary team.Commitment to ongoing education and the provision of evidence-based practice.Sound knowledge of local service provision including those through My Aged Care and NDIS.Knowledge of hospital operations and processes including discharge planning.Knowledge of EPOA, QCAT, adult guardian requirements and responsibilities.Availability for Monday, Wednesday, Friday work.Applications close: 22nd October 2024
