Role:
Based in ACT, your responsibilities will include:
Providing comprehensive services to inpatients (including inpatient rehab, orthopaedics, general surgery, cardiac and neurosurgery, and ICU/CCU patients).Delivering excellent clinical services related to assessment, treatment, planning and evidence-based practice.Participating in program development and quality improvement activities within the Department.Working closely with the multidisciplinary team to provide comprehensive care to patients and their support people.Discharge planning with the multidisciplinary team and referrals to external services.Selection Criteria:
Tertiary qualifications in Social Work, with eligibility for AASW membership.Demonstrated experience working in a hospital setting.Experience and skills to conduct client assessments, and develop care plans that support clients' needs.Excellent organisation, time management and self-management skills.Highly developed communication skills and interpersonal skills.Commitment to ongoing education and the provision of evidence-based practice.Additional Information: Part-time roleHospital settingAvailable immediately #J-18808-Ljbffr
