We are seeking applications for a Social Worker (Grade 2) to join the Mental Wellbeing team on a part-time, fixed term contract until 30th June 2026, working across Daylesford, Creswick, and Clunes sites.
Role Statement: The Social Worker will provide services to community clients, residential aged care residents, and hospital in-patients, collectively known as consumers. The role is to assist and support consumers, their families, and support networks in response to physical, emotional, and social factors that negatively impact the consumer's health and wellbeing. The social work role also has a primary and secondary prevention component focused on averting the onset of poor mental and physical health and early intervention by addressing social and environmental determinants of health.
Details of Appointment: Part-time, fixed term appointment until 30 June 2026, working up to 76 hours per fortnight across Daylesford, Creswick, and Clunes sites.
Dependent on experience, as per Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 award + superannuation.
