Position: Permanent Part Time - 0.8EFT
Classification: Grade 2 Social Worker (SC21-SC24)
Department: Cardiac Heart Failure and Cardiology - Alfred Hospital
The DepartmentThe Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.
The RoleThe Social Worker in CAHF and Heart Transplant is responsible for the provision of advanced social work services to patients and their families/significant others within the Alfred Heart Transplant and Cardiac Heart Failure Service. The worker will be required to consult with colleagues and staff from other disciplines on relevant issues as required and to provide advanced casework expertise. The worker will be required to complete comprehensive pre-transplant assessments using an assessment tool - The Stanford Integrated Psychosocial Assessment for Transplant (SIPAT). The worker will also work within the Trauma stream and will provide support to Social Work within designated units allocated within the Trauma stream by the Trauma Team Leader. An understanding of Cardiology and psychosocial issues evident in this patient cohort will be desirable for this role. This position may be required to work at any or all campuses of Alfred Health as and when required.
Skills/Qualifications RequiredEssential: Eligibility for membership of the Australian Association of Social Workers.Bachelor of Social Work, or recognised equivalent.Ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities.Required to perform work that requires special knowledge and depth of experience in any one or more of the following: Family Violence, Child Protection, NDIS.Knowledge of current legislation and policy in relation to Social Work in a hospital setting.Desirable: Working towards or postgraduate qualifications in Social Work or a related field.Hospital-based work experience.Sound IT Skills.Staff BenefitsSalary Packaging and Novated LeasingDiscounted Health InsuranceStaff Car Parking (subject to availability)Onsite Gym (Alfred Hospital)Close to shops and public transportFor more details on the role please download the attached Position Description.
