Your opportunity: The purpose of this virtual (admitted) service is for subacute and non-acute patients who would benefit from the opportunity to improve their functioning while in their own homes. The position requires working with a specialised team to facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community. This approach aims to reduce prolonged in-hospital patient admissions (extended stays). Deliver high-quality core Social Work services as a multidisciplinary team member to the Wide Bay Hospital and Health Service patients across two hospital campuses and their families and carers to optimise their psychosocial outcomes and achieve maximum benefits from health care services. Prior to being made an offer of employment, you will be required to demonstrate immunity to the following Vaccine Preventable Diseases (VPD): Hepatitis B, Measles, Mumps, Rubella, Varicella (chicken pox) and Pertussis (whooping cough).
