Community Rehabilitation Program - Social Worker Department : Community Rehabilitation Program
Status: Max Term Part Time (Maternity Leave Coverage)
Applications are sought from suitably qualified clinicians to work in the Community Rehabilitation Program at Echuca Regional Health.
Key responsibilities include:
Provide sound assessment, care planning, support and encouragement in working toward agreed goals as determined by a comprehensive assessment and care plan;Provide comprehensive and responsive Social Work service to clients, their families and significant others and facilitate optimal adjustments to the impact of change in health status on individual and family functioning;Provide responsible, professional and timely assistance and support to clients, their carers and family members concerning the process involved with ACAS assessments, community services and Residential Aged Care Facilities;Liaising with relevant health professionals and within the interdisciplinary team to ensure optimum medical and therapeutic management of each client, safe program delivery and timely client referral to additional health and community services;Our team requires someone who has:
Qualifications in Social WorkDemonstrated ability to practice collaboratively as part of a transdisciplinary health; care team and autonomously as requiredDemonstrated provision of high-quality client care;Developed assessment, clinical reasoning, problem solving and prioritization skills;High level of reliability and professional conduct;Excellent communication and interpersonal skills;High motivation and willingness to learn.This position requires completion of a satisfactory National Police Check and (where relevant) provision of a Working With Children Check and check on the Disability Worker Exclusion Scheme prior to appointment.
