Your Opportunity Provide comprehensive support and advocacy to clients and communities grappling with Alcohol and Other Drugs (AoD). You will engage in providing case management and case co-ordination to individuals with drug and alcohol needs including housing, parenting and family support, income and employment. You will be required to be creative in addressing issues and challenges that are faced by participants in order to achieve the desired outcomes.
Your key responsibilities will be to:
Provide intensive, evidence-based, person-centered case management and review progression against case plan. Deliver treatment interventions aligned with NSW Quality Framework and Clinical Care Standards for Alcohol and Other Drug Treatment through counselling, care coordination and education sessions and group sessions. Work with clients to create holistic and strength-based support plans and facilitate AOD education sessions and awareness within the community. Respond to referrals of clients to the service from other services and/or direct client contact. Assist program manager and other staff in addressing complex cases or emergencies. Complete a range of administrative duties for the efficient running of services. Actively participate in staff meetings, team discussions and relevant network meetings. Support Clinical governance framework and implement quality program activities to ensure compliance with service standards. Requirements for Success Degree/Diploma in Social Work with at least 2 years' experience working with mental health and/or AOD program. Experience providing case management to individuals with complex needs through trauma-informed practices. Understanding of substance abuse disorder and/or mental illness. Well-developed written and verbal communication along with interpersonal communication. Strong computer skills and understanding of specialized programs e.g. word, excel, PowerPoint. Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people and child-safe certificate.
