Join our skilled team as a Social Work Assistant where you will provide exceptional patient-centred care to health services consumers.The Social Work Assistant works as an integral part of the Social Work team, providing clinical support tasks under the direction and supervision of an allied health professional.
About the role:To contribute to patient care by undertaking support tasks delegated under the supervision of a Social Worker.
The role supports Social Workers in their day-to-day clinical and operational duties, to optimise patient outcomes.
Under the direction of Social Workers, perform tasks including (but not limited to):
Assist patients and their families with completion of forms and paperwork related to applications for residential aged care.Assist patients and their families access hospital services, e.g.
organisation of accommodation and travel.Communication and liaison between Social Workers, Patients, Families, and Carers.Liaison with external providers including Aged Care Facilities and services, Community organisations, and government services.About you:Non-mandatory qualifications/requirements: While not mandatory, possession of or in the process of obtaining the following is desirable:
Certificate III in Health Services Assisting, orCertificate III or IV in Allied Health Assisting, orCertificate III in Health Administration, orCertificate III in Community Services Work, Aged Care, Disabilities or similar.What we offer:Salary information: Total remuneration value of up to $82,130 p.a.
