Social Work- core 28hrs per week up to full time The Social Work position provides clinical assessment and interventions to support Carers and volunteers of the Palliative Care and Dementia Support programs. Focus upon emotional and practical support to lessen the burden of care and help identify strategies for self- care, enhancing the ability to continue caring within their family member with a life limiting illness at home. This position requires travel to and from client's homes as well as support to clients in the community, vehicle and cleaning equipment provided.
