As a Social Support Worker, you will play a vital role in facilitating our Social Support programs, providing essential services to frail, aged, or permanently disabled individuals and their carers.
You will collaborate with a passionate team to design and implement diverse activities aimed at promoting wellness, reablement, and community participation.
From gentle physical activity and exercise groups, engaging centre-based activities, opportunities for celebration, to social interaction and fun and varied community outings, your creativity and dedication will help empower our participants to live fulfilling lives.
Key Responsibilities: Plan and facilitate diverse activity programs tailored to meet the needs and interests of our participants.Provide advocacy, support, and information to empower individuals and their families to access relevant services and resources.Collaborate with team members and external stakeholders to ensure holistic and coordinated care for our participants.Maintain accurate records and documentation in accordance with organizational policies and funding requirements.Uphold Dhelkaya Health's values of integrity, compassion, and respect in all aspects of your work.As a valued member of our Social Support & Wellbeing Team, you will have the opportunity to contribute to the well-being and quality of life of our diverse community of participants - this is a dynamic role where every day is a chance to make a positive impact on people's lives.
About you What we're looking for:
Experience undertaking group activity programs, especially with older adults and individuals with disabilities.Strong consultation, planning, and facilitation skills to create meaningful and engaging activity programs.Excellent communication skills, both verbal and written, and the ability to work collaboratively within a team.A commitment to delivering person-centered care that promotes independence, connection, and meaningful engagement.This position requires a Certificate III in Aged Care or Disability Services, demonstrated experience in supporting aged and frail or disabled individuals, the ability to work both independently and collaboratively within a team, and proficiency in providing recreational and support services in a group setting.
A valid driver's license is also an essential requirement for this role.
