SOCIAL SUPPORT GROUP COORDINATOR & CHSP INTAKE OFFICERITSOWEL - Aged and Community Services – Wollongong NSW
Providing direct leadership, support, and direction to Social Support Group Facilitators to ensure a high level of care and service in line with Government guidelines and the Aged Care Quality Standards.
Coordinating Social Support Group activities, including transport and reassessments of clients.
Ensuring that a meal program is planned and monitoring food safety guidelines and policies.
Ensuring all consumer records are up to date.
Responsible for the timely intake of CHSP consumers from the My Aged Care Provider Portal and initial contact and assessment.
Working closely with the Home Care Team to ensure timely commencement of service/s and follow up.
Leading the CHSP program through the transition to Support at Home program.
