See more at our website - www.atsichsbrisbane.org.au Job DescriptionAbout the Role The Social Health Care Connect Practitioner effectively provides early intervention, crisis management and aftercare services to clients referred to the Social Health Culture Care Connect program. They will be able to contribute to providing access and coordination to comprehensive support services for clients and their families, with the aim to achieve personal wellbeing as part of a holistic model of care. They can contribute and participate in a diverse and multi-discipline team that promotes a flourishing workplace culture and upholds the ethos and values of ATSICHS Brisbane as a long-standing Aboriginal Community Controlled Organisation.
Duties Intake referrals and provide quality support.Data management.Appropriately respond to critical incidents, providing wellbeing first aid to clients in distress.Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.Provide integration and assisted referrals and coordination between ATSICHS services to facilitate the delivery of an efficient, responsive, and integrated mental health and social needs service to patients.Liaise with other community organisations and government agencies for the purposes of information gathering, referral, networking and promoting ATSICHS Brisbane services.Self-management and team contribution.Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff.Culture Enjoy working in an organisation that makes a difference in the community. Annual salary $82,964 per annum.11.5% Super + Not for Profit Salary Packaging (Increases your take home pay).Work for a leading Aboriginal and Torres Strait Islander, community-controlled organisation.Supportive and collaborative team of dedicated professionals.COVID-19 Requirement Immunisation history showing you have received two doses of a Covid-19 vaccine is encouraged.Desired Skills and ExperienceRequired Tertiary qualification in Social Work or Human Services or related field and/or demonstrated experience in providing support and case coordination to people with complex needs.Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.Ability to work with diverse people and presentations in providing case managed social work driven supports.Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.Demonstrated ability to record accurate and consistent case notes.Demonstrated IT capability - experience in computer-based data management systems and Microsoft Office.A willingness to travel across the ATSICHS Brisbane region of services and work across different sites.A willingness to provide social work supports and engagement to patients outside of a clinic environment.Working with Children's Card (Blue Card) and NDIS Worker Screening (Yellow Card) or be eligible to apply.Preferred Previous experience in a multi-disciplinary team environment.Previous experience working in an Aboriginal Medical Service.How to apply If this sounds like you, please submit your application by following the links on the recruitment page.
