The role will ensure that clinical work is carried out effectively from intake to transition, liaising with various stakeholders and the team. The role requires great customer service and a hands-on approach, whilst actively maintaining a positive and long-term relationship with external relations and handling any concerns that may arise.YOUR RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:Provide complex person-centred case management, including assessment and care planning;Ensure clinical aspects during the intake and transition process is overseen and well prepared;Prepare detailed reports, or provide data within the area of your responsibility for the Company's use;Contribute to the development and implementation of systems that ensure Support Staff are able to deliver desired outcomes for participants, including systems to support planning, budgetary, resource management, risk management and quality assurance functions, within the role's scope;Interpret participant data, including whole system analysis, to identify appropriate actions to meet participant interests at a whole service or group level;Lead complex negotiations and discussions with family members, medical services, activity services, police and other community services to support planning for future participant requirements and to negate any possible risks;Keep accurate and complete records of work activities in accordance with the relevant legislative requirements, the Company's records and information security and privacy policies;Provide participants with high quality operational support and ensure good relations are maintained, and inquiries are investigated and resolved efficiently;Provide continuous supervision, support and mentoring to Support Staff;Manage Support Staff;Monitor Support Staff performances and relevant processes (e.g., signing in and out, progress notes)Ensure Support Staff have a platform to openly communicate and/or debrief with SIL Team Leader;If and when necessary, respond to calls and messages after business hours;Prepare rosters and schedules to support participant requirements.ESSENTIAL CRITERIA:Understanding of the NDIS and Disability IndustryCurrent AHPRA registration in Registered NursingMinimum two years' experience in Clinical Nursing experienceWilling to work outside of standard office hoursDriver's License with access to a carIntermediate in Microsoft OfficeExcellent time management skillsStrong analytical and decision-making skillsAbility to build rapport and trust with participants and stakeholdersExcellent written, verbal and interpersonal communication skillsStrong project management and multitasking skillsAbility to work in a fast-paced environment, can accomplish tasks without direction.Job Types: Full-time, Part-timePay: $38.00 – $59.00 per hourSchedule:Shift workSupplementary Pay:Performance bonusLicence/Certification:First Aid Certification (Preferred)Working with Children Check (Preferred)National Police Certificate (Preferred)Driver Licence (Preferred)#J-18808-Ljbffr
