About the Role: As the Service Planning Coordinator, you will be largely responsible for understanding and meeting the support needs of new and existing members.
Your role will be crucial in supporting the organisation with its growth targets, whilst building meaningful and lasting relationships with members and their families.
You will also: Lead and provide excellent customer service across support services to facilitate positive member outcomes.
Develop Individual Support Plans (ISP) for all members utilising the organisation's Support Worker Services Manage customer and member enquiries face-to-face, over the phone and via email with a positive and empathetic attitude Ensure all documentation, communication and administrative needs meet the standards needed to support members across our services Track member's progress towards goals and outcomes, working with both internal and external stakeholders to support these.
Undertake business development strategies, with the intent of implementing retention, satisfaction and utilisation strategies.
Ensure member's matters are addressed, triaged, and managed within a reasonable time frame.
Perform some incentivised on-call duties when required and necessary A Bit About You: To have success in the role you will be a driven, confident and autonomous worker with the ability to drive person-centred planning and customer service within the NDIS sector.
You will also have: Minimum one-year prior experience in the NDIS sector, working across planning or a similar client management role Strong understanding of NDIS, NDIA and DSOA processes and provider requirements A proactive, person-centred approach to customer service that is conducive to supporting the delivery of member outcomes Strong ability to work autonomously and self-manage Excellent written and verbal communication skills, and analytical and report-writing skills The ability to both manage and support individuals' expectations and is comfortable handling people with complex and demanding needs The ability to manage a number of competing tasks and priorities concurrently, working in a high-pressure and high-volume work environment A positive attitude and ability to respond to challenges with a growth mindset and resilience Valid NDIS Worker Screening Check, Working with Children Check (WWCC) and Driver's Licence What's In It For You: Join an organisation with high member satisfaction, and an excellent culture, with a clear vision, mission and purpose.
