About the role: We are seeking a highly skilled and experienced Service Manager to join our Home Care team. In this role, you will be responsible for delivering all in-home and centre-based services as funded through Commonwealth Home Support Program, Home Care Packages and various other funding sources, as appropriate within the local community, ensuring a positive outcome for our clients.
Key responsibilities include:
Leading and supporting a multidisciplinary team Monitoring and reviewing the delivery of services to meet our clients' individual needs Undertaking regular risk assessments and seeking feedback with a continuous improvement approach Managing organisational efficiencies, including scheduling, productivity, and operational costs Quality and compliance relevant to the ACQSS About you: Our ideal candidate will have significant experience in a similar role, with a proven track record of meeting objectives and timelines. You will have a passion for promoting independence and self-determination in older people. Collaborative partnerships are important to you, and you are skilled at building and drawing on mutually beneficial stakeholder relationships at all levels.
