We are funded by the Australian Government to help people in South East Melbourne get the health care they need, when and where they need it.Job SummaryReporting to the Manager Provider Support, the Service Coordinator role will be a part of a team working on Aged and Primary Care contracts and projects. This exciting newly created position is due to additional Government funding to support dedicated activities for better health, wellbeing and primary care for senior Australians within our catchment.Your primary role will be to deliver the General Practice Aged Care Incentive (GPACI) as one of the new Strengthening Medicare initiatives. The program aims to support every aged care resident in Residential Aged Care Homes (RACH) to receive quality and continuous primary care services from a regular General Practitioner (GP) and practice or Aboriginal Community Controlled Health Service (ACCHS) depending on residents' preferences.The Role of the GPACI Service Coordinator will be to:Work in collaboration with residents (and/or their nominated guardians), RACH providers and practices in the region to develop processes that match RACH residents with an appropriate preferred GP in MyMedicare where they do not currently have one.Establish collaborative, reciprocal, and formal arrangements with local Aboriginal Community Controlled Health Services (ACCHS) and identify and/or develop culturally safe and appropriate pathways for First Nations RACH residents who request healthcare from within the PHN region.Identify and share examples of best practice arrangements between practices, GPs, ACCHS and RACHs through establishment of networks to improve capacity.Increase knowledge of GPs, practices and ACCHS of the GPACI and its benefits to residents of RACHs and providers.Improve reciprocal relationships between GPs and/or ACCHS and RACHs to facilitate the delivery of quality and continuous primary care services.Improve the capacity of GPs, ACCHS and practices to deliver quality and continuous care through collaborative learning networks and formalised arrangements such as Memoranda of Understanding, and the embedding of the Best Practice Guidelines (to be developed by the Department in 2024).You will be a member of a team which works on several contracts/projects simultaneously; the ability to plan and prioritise will be essential.The role will work collaboratively with stakeholders and consumers to inform and support solutions. Your expert coordination and stakeholder management skills will be vital to the effective roll out of these programs. In addition, the role may be required to provide project support to other SEMPHN initiatives.The role will also be responsible for active oversight of contracts for commissioned services whilst supporting stakeholder engagement, implementation of SEMPHN's commissioned programs and assisting in the development of evaluation tools to monitor performance.Your strong problem-solving ability and eye for detailed coordination will promote success in this role, whilst supporting the team as required.Skills, Knowledge and ExperienceEstablished contract management skills including administration.Demonstrated experience in health and community service programs.Experience in working with a range of health care providers including General Practices, nurses, Allied Health Providers and consumers.Strong problem-solving skills with the ability to analyse issues and provide advice based on relevant and applicable literature and data.Demonstrated positive stakeholder engagement and relationship management skills to support effective systems change with improved health outcomes.Please review the position description attached below for further details about the role.
