We are currently seeking a dedicated Disability Service Coordinator with expertise in rostering to join our team.Key Responsibilities:Service Coordination: Develop and implement individualised service plans for clients, coordinating with families, healthcare professionals, and internal teams to ensure comprehensive support.Rostering: Create, manage, and adjust staff rosters to ensure adequate coverage for client needs, balancing staff availability, client requirements, and compliance with relevant regulations.Client Advocacy: Serve as a primary contact for clients and their families, addressing any concerns, providing information, and advocating for their needs within the service framework.Quality Assurance: Monitor service delivery to ensure adherence to organisational standards, legal requirements, and best practices in disability support.Documentation: Maintain accurate records of client services, staff schedules, and any incidents or changes in client needs or staffing requirements.Problem Solving: Address and resolve issues related to service delivery or rostering efficiently and effectively, adjusting as necessary.Collaboration: Work with internal teams, service providers, and community organisations to coordinate services and ensure a seamless client experience.Training and Support: Provide support and training to new staff members, ensuring they understand their roles and responsibilities in relation to client care and scheduling.On Call: Some ability to be on call on a rotating roster.Qualifications:Formal qualification in Disability Services, Human Services, or a related field desirable.Proven experience in disability services, case management, or a similar role, with a good understanding of rostering and scheduling.Excellent organisational skills, with the ability to manage multiple priorities and adapt to changing needs.Strong communication and interpersonal skills, with a focus on empathy, client care, and team collaboration.Proficiency in rostering software, Visual Care, and other relevant applications can be learnt.Knowledge of disability services regulations, best practices, and community resources.Ability to work both independently and as part of a team.How to Apply:If you are passionate about making a positive impact in the lives of individuals with disabilities and have the expertise in coordination and rostering, we would love to hear from you.
