We know that choosing the right carers and support team can make a huge difference to wellbeing, so we'll genuinely listen to NDIS participants' goals, support and empower our staff team to achieve positive outcomes that will help participants live a happy and fulfilled life supporting independent living and community participation.aspiring futures SA is seeking a Service CoordinatorThe Service Coordinator will be responsible for the operational management of their portfolio of NDIS participants & support team and is responsible for the effective management of various high-quality and individualised services being delivered to NDIS participants while promoting their choice and control.The Service Coordinator will lead by example to coach and mentor your team's day-to-day activities and work closely with participants and their families to ensure excellent and high-quality service is delivered.Our ideal candidate will be:Great at building relationships with our participants, their families, support workers, peers, other services and stakeholdersTenacious and have a can-do attitude to solving challenges and making a difference in the lives of the participants we supportKeen to get involved.The successful candidate will be required to clear all state-based probity checks, including but not limited to, NDIS Worker Screening Check and Working with Children Check.SA Driver's Licence and evidence of COVID vaccination are required.Key Responsibilities of the Position:Ensuring all support services are delivered in line with the participant's needs and personal goals.Promote effective liaison/communication with participant's needs, families, support team, and relevant stakeholders.Develop a high-performance culture, committed to quality and outstanding service to the participants.Lead and manage teams to achieve participant's goals.Provide leadership in delivering high-quality life outcomes for our participants (and their families where appropriate).Provide management support and practice leadership to support teams within their area of responsibility.Promote participants-directed support that exceeds the minimum NDIS standards and is delivered within the available resources.Play a key role in improving efficiencies and implementing new ways of working across the team.Maintain knowledge of NDIS incident reporting, and reporting of URP and RP.Ensure the implementation of BSP/work with the allied health team of the participant to work continuously towards the participant's NDIS goals.Ensure effective operational management of the portfolio including all administrative support within policy and procedural guidelines.Establish and maintain collaborative relationships with key internal/external stakeholders.Be responsible for the overall performance within their area of responsibility.Your application will be assessed against the following Mandatory Requirements:Formal qualification/s in a Human Services discipline or equivalent demonstrated experience in managing service delivery in a disability/community services environment.Solid knowledge of the NDIS funding model and processes.A high degree of personal resilience, demonstrated through the ability to work through difficult issues whilst remaining professional and keeping your eyes on operational change and performance improvement.Have excellent problem-solving skills, be decisive and willing to "have a go" at managing difficult situations.Ability to manage staff resources in a complex environment.Understands legislation and standards applicable to the disability sector.Understands risk management principles.Excellent written and verbal communication and negotiation skills.Excellent computer literacy including Microsoft 365 with the ability to learn and manage new IT systems.Strong problem solving and decision-making skills.Proven ability to work well as part of an innovative team as well as work independently as required.Excellent organisational, time, task, planning, and priority management.Passion for supporting the human rights of people with a disability and a focus on quality outcomes.You will have a strong background in working in NDIS Disability Services or similar, most likely from the disability, health or community services fields.
Strong leadership and administrative skills, as well as the ability to multitask on a constant basis, are key requisites of the position.
