Sovereign Lives is on the lookout for a dedicated and efficient Service Coordinator to join our growing team.
Sovereign Lives assists with the delivery of care and support services to people with disabilities to continue living independently at home and in the community.
We are passionate about supporting people with disabilities, and that means providing advice, empathy, and understanding to ensure that our amazing participants have the support they need to live happy and healthy lives.
About the role: Service Coordinators plan and coordinate the delivery of an organisation's services.
They build and maintain relationships with Disability Support workers, NDIS participants, families, allied health, and other stakeholders to continuously improve the level of service by ensuring that the participants' requirements and organisation's goals are met.
Service Coordinators are highly organised and have strong administrative and communication skills.
They interact with participants daily and use a range of computer software.
They act as an advocate for the NDIS participants and provide information on how they can access the community services.
Additionally, they work as an intermediary between participants, often serving in a support role for them during difficult times.
Employment type Part-time- 25 - 30 hours a week
Monday to Friday: 9am to 3:30pm
Effective immediately.
Responsibilities: Working directly with clients, family members, and their supports to identify their needs and ensure that the support provided meets them.Updating Care Plans and Budgets for clients to ensure that their care needs and deterioration of health are well documented.Providing support from your Manager and team members to achieve goals.Contributing to building a warm and friendly work environment.Conducting and attending meetings and training sessions.Educating people with disabilities and family members/health care workers about health, safety, and quality living standards.Preparing care plans/reports of NDIS participants and briefing health care workers.Conducting training and briefing sessions.Working under the management of Senior Management members.Working in a team environment by diverting or receiving phone calls during colleagues' absence.Promoting the services of the organisation based on your network building and excellent care coordinating services.Preparing and writing weekly/fortnightly or monthly reports of participants' services.Writing progress notes in our established CRM and system.Providing accountability of accomplished/unaccomplished tasks, exchanged phone calls, messages, and progress with all participants.Maintaining privacy and confidentiality of the organisation.Working according to the NDIS Code of Conduct.Coordinating with the rostering team to ensure that participants are receiving supports as and when required.Maintaining an Excel sheet of all compliant documents (incident forms, PCP, ERM, Home Risk assessment, and more).Using multiple CRM platforms and One Drive as a storage platform.About you: This position would be suitable for someone with a medical background/public health, or someone with experience in residential and/or community care.
