Competitive salary + super + $18,550 salary packagingBased in ParramattaThe Service Coordinator will provide service coordination for care recipients receiving Bolton Clarke Home Care Packages funded by the Commonwealth Government, Department of Human Services.
What are the Home Care Packages?Aligned with the Consumer Directed Care framework, the key aims of a Home Care Package is to assist people in living at home for as long as possible, enabling them to have the choice and flexibility in their at-home support and assist consumers and providers to maximise this choice and flexibility.
As the Service Coordinator for Home Care Packages, you will have a wide knowledge of community resources, ensuring care recipients receive high quality, seamless and cost-effective care and services.
