The Service Coordinator will provide service coordination for care recipients receiving Bolton Clarke Home Care Packages funded by the Commonwealth Government, Department of Human Services.
What are the Home Care Packages?

Aligned with the Consumer Directed Care framework, the key aims of a Home Care Package is to assist people in living at home for as long as possible, enabling them to have the choice and flexibility in their at home support and assist consumers and providers to maximise this choice and flexibility.
As the Service Coordinator for Home Care Packages, you will have a wide knowledge of community resources, ensuring care recipients receive high quality, seamless and cost effective care and services.
The Service Coordinator role will: Receive calls routed from the Customer Service Centre in relation to customers/external providers queries and responded to in a timely mannerRespond to calls appropriately within scope of role and business rulesDevelop and maintain relationships with service providers to ensure service coordination and all relevant client information is obtained accuratelyEnsure good working relationships with relevant stakeholdersRespond to all communication from internal/external customers in a timely, courteous and professional mannerBe a central point of contact for team via Customer Service Centre The Service Coordinator will provide service coordination for care recipients receiving Bolton Clarke Home Care Packages funded by the Commonwealth Government, Department of Human Services.

Excellent communication written and verbal skillsExcellent computer skills with demonstrated experience in MS OfficeDemonstrated commitment and experience in providing high quality customer serviceA strong customer focused approach to careProfessional telephone mannerExcellent organisational and time management skills and ability to work within limited time framesPolice Check (Valid within the last 3 years)Drivers LicencePrevious experience in a similar role within community/aged care would be an advantageWhy should you join Bolton Clarke?
