Job DescriptionAbout the role The Service Coordinator is responsible for developing, promoting and implementing the Transition to work, Step up, Supported Independent and Individualised Support programs for clients within Murray Bridge region and in other community settings.
The incumbent provides client-centered leadership to a team and facilitates teamwork to enable the delivery of safe and effective, individualised, person-centered supports for people living with disability to assist them to achieve their goals.
The position contributes to Novita services more broadly by collaborating with other Novita staff and the community to support people where they want and facilitate staff movement across the range of support types to meet client needs.
This is a full-time ongoing opportunity based in Murray Bridge.
This position is paid in accordance with the SCHADS award, Level 4 along with Salary Sacrifice option.
As a Service Coordinator, you will:
identify the needs of clients and respond by linking them to existing services and developing new servicesdevelop and teach a program of activities which is engaging, offers opportunities for skill development, developing independence and forming relationships through community participationlead a team of Senior Support Workers and Support Workers as the programs grow, allocating and organising staff supports for clients in line with their funded model of care and support requirementsprovide supervision and coaching to the Senior Support Workers and Support Workers in the team to enhance their practice and the outcomes for clientsactively identify and manage risks and incidents when they occur to support the safety of both clients and staffdeliver quality service improvement in line with the 'Stepping into the Future' project and SCERTSdrive operational performance to deliver service, financial and business goals.Desired Skills and ExperienceAbout you As well as being aligned with Novita's ICARE values, you bring:

All Novita employees are required to undertake mandatory training relevant to their role.
Service Coordinators are paid in accordance with the Social, Community Home Care and Disability Services Industry Award 2010 (SCHADS).
