About this Role:
As a Service Coordinator you will be the primary point of contact for our Aboriginal Home Health customers across the Coffs Harbour regions of NSW. This is a full time fixed term opportunity where your key objective will be to manage a portfolio of customers receiving In-Home care.
You will enjoy being active in your local community meeting with clients and their families.
Some of your role responsibilities:
Ensure excellence in client service delivery, including managing the assessment of client care and goal planning and the successful on-boarding of new clients. Managing client escalations and complaints to a high level of resolution. Successfully converting new client leads into active customers Developing exceptional relationships with both existing and new clients and their families. Developing effective collaborative relationships with other relevant service providers, including Community Support, Clinical and Allied Health services. About You: Does this sound like you?
An understanding of the needs of Aboriginal and/or Torres Strait Islander communities Previous experience managing a service area or program within a Health Care, Aged Care or Community Service organisation Experience managing a high-volume client caseload, including complex client care needs, service allocation, compliance, and case management Some knowledge of Home Care Packages would be highly beneficial Australian drivers licence required Vaccinated for COVID-19 (2 doses) Benefits include the following:
