Summary Work within a values driven, progressive organisation Competitive remuneration package with salary packaging benefits available  Be part of an engaging team that thrives on challenges About the Opportunity The Adult and Family Homelessness Service (AFHS) and the Family Crisis Accommodation Service (FCAS) provide transitional, and crisis support to families and single adults across the Western Region.
The programs provide a mix of short- and long-term case management to people experiencing homelessness who have been referred by the two Western Homelessness Access Points.
The focus of both programs is on assisting people to access and maintain long term housing.
Recently AFHS saw two gaps in service availability within the local government areas of Melton, Maribyrnong and Brimbank.
Specifically, a need to support people sleeping rough in Melton, along with people who were being discharged from hospitals with multiple medical conditions into homelessness in the greater West of Melbourne.
This resulted in the Melton Rough Sleeper initiative and the Hospital Discharge project being established.
These projects within AFHS are designed to address gaps, navigate new pathways through existing barriers and provide service delivery to people most at need and not just the help seeker's that present for assistance at the traditional access points.
The Senior Worker - Melton Rough Sleeper and Hospital Discharge is currently a maximum term role until December 2025 within the Adult and Family Homelessness Services program that is responsible for holding a small caseload along with being the lead coordinator of both initiatives.
Melton Rough Sleeper - The Rough Sleeper project has been running for 18 months and now requires a review of its operations and time to collect and analyse data for a business case to attract future funding.
Currently, MCM relies on the ability to access limited funding for emergency accommodation and material aid, which is not sustainable.
Hospital discharge- Our Hospital Discharge project is currently working with two hospitals.
To establish this program effectively, we need a Senior Worker to set up processes, build networks, and implement data collection.
This role is crucial for piloting the program, attracting future funding, and potentially expanding to more hospitals.
The role, as part of a larger case management support team, aims to provide guidance and leadership around short term intensive and holistic support to achieve housing stability and broader goals.
Service delivery will be consistent with MCM's Healing Orientated Framework, and trauma informed practice.
About you We are looking for someone who has: Experience in leading a team of professionals working in homelessness context, with the ability to oversee a range of and case management strategies to the people we support Demonstrated ability to lead, guide, support employees with a sound understanding of best practice principles Demonstrated experience in developing, delivering and evaluating high quality programs and services Extensive knowledge of casework principles Benefits of working with MCM include: Diverse and inclusive organisation Attractive Salary Packaging benefits Supportive learning and development environment Culture of continuous improvement Essential Bachelor of degree in Social Work, Community Services or a related discipline with experience working with people having an episode of homelessness and at least 5 years in a similar case management role Sound theoretical practice in working with people experiencing homelessness and complex case management based on best practice and current theory.
Combined with an understanding and experience with the homelessness sector and/or mainstream services, including knowledge of patterns, trends and systemic issues Demonstrated ability to contribute to the team to create an innovative response that disrupts disadvantage and leads to sustainable housing first outcomes for people A clear understanding of, and ability to lead case management, including client assessment and intake processes Demonstrated ability to work collaboratively with other organisations and welfare sectors, in formal and informal partnerships, to achieve client outcomes Demonstrated well-developed written and verbal communication, interpersonal, conflict resolution, negotiation and mediation skills Ability to build and maintain positive relationships, and communicate with people of diverse backgrounds and abilities Computer literacy, including proficiency in using databases and client management systems Screening Criteria National Police check Working with Children check Valid Victorian Drivers license Right to Work in Australia Applications for this role will close on 22 November 2024 and applications may close before the deadline, so please apply early.
