Position Profile: Plan, coordinate, deliver and evaluate timely, culturally appropriate Social Work services for Fitzroy Crossing and surrounding communities at an advanced practice level. Using a primary health care approach, contribute to promotion, maintenance and improvement of health and well-being of clients and families within Fitzroy Crossing and surrounding communities. Providing clinical services for clients over the lifespan in both community and acute settings. Assists with discharge planning and coordination of care to promote optimal health outcomes for clients with complex psychosocial needs. Provides secondary consultancy services and support to WACHS and other staff working across the region.
