About the role
Fixed term temporary part time opportunity to deliver high-quality social work clinical services as part of the multidisciplinary team to patients of the Central Foot Hub managing a varied and complex caseload of patients with foot disease and high-risk foot in both inpatient and outpatient settings, virtual settings, identifying clinical risk and providing multiple clinical interventions to maximise patient health and psychosocial outcomes.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Deliver high quality evidence based clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards by assessing complex psychosocial needs of patients, families and caregivers; identifying clinical risk and providing multiple clinical interventions to maximise patient health and psychosocial outcomes. Provide high level expertise in the delivery of social work services to clients through collaborative participation as a multidisciplinary team member and contribute towards multidisciplinary team processes. Contribute to the ongoing identification, development and implementation of quality and service improvement activities that enhance the delivery of social work services, patient care. Health Equity
