About the Role As the Senior Practitioner Home Based Care, you will play a pivotal role in supporting the coordination and delivery of Home Based Care services, including Therapeutic Foster Care within the Foster Care and Kinship Care programs. Working from a trauma-informed lens, you'll manage a complex therapeutic caseload while providing mentorship and guidance to practitioners. You'll apply a range of frameworks, such as the Best Interest Case Practice Model and Circle Therapeutic Foster Care, to ensure that children, young people, families, and caregivers receive the highest standard of care.
To be successful in this role you'll need to build trusting relationships, understand the unique challenges faced by clients, and work closely with the Team Leader to enhance service delivery. Your expertise in risk assessment, trauma-informed care, and legislation will be key to fostering positive outcomes. Additionally, you'll collaborate with a diverse range of stakeholders and community partners to drive meaningful change and improve the lives of vulnerable children and families.
As our Senior Practitioner Home Based Care, you will be required to work autonomously and as part of a team, maintain accurate records, and actively participate in and contribute to program service planning, along with all annual improvement activities to achieve organisational outcomes. It is expected that this role will maintain a complex client load as well as mentoring and supporting other workers within the team.
Key Responsibilities Provide Therapeutic Foster Care to Circle accredited households, children, and young people. Coordinate and administer the programs in accordance with program service plan, service agreement, and funding body requirements. Guide and mentor team members in relation to case practice, care teams, and contracted case management. Undertake case work and case contracting of complex children, youth, and families. Do your skills include: Ability to hold a complex caseload and work therapeutically with children and young people in Out of Home Care. An understanding of child development and factors that can impede a child's development, including early indicators and patterns of behaviour leading to cumulative harm. Ability to make timely, sound decisions about interventions required, assessment, and decision-making in the best interests of the child/ren. Demonstrated knowledge and understanding of appropriate legislation relevant to the role, case management practice, and the ability to adhere to policy and procedures. Are your credentials: A minimum qualification of a sector recognised and accredited Bachelor of Social Work or Diploma in Community Welfare Work or equivalent. A current Drivers Licence.
