This is a Recruitment Pool for Permanent and Fixed Term appointments, with the possibility of extension(s) and/or permanency. Hours negotiable. Position Profile:  We are seeking a suitable candidate to start as soon as possible on a part-time basis (0.5 FTE) for 12 months to lead a rationalisation project for erythropoietin stimulating agents for renal patients.
Additionally, we are seeking a pool of suitably experienced candidates to lead the development of renal pharmacy services in WACHS.
This position provides advice relating to the safe, rational and cost effective use of medicines for country renal patients as part of the Regional Renal Support Teams and visiting nephrology service. Successful candidates will be expected to liaise with key multidisciplinary team members to ensure a holistic approach to the care of patients receiving care for Chronic Kidney Disease; and plan, implement and evaluate patient safety and quality improvement initiatives including development of clinical guidelines and policies.
This position ensures practice is in accordance with the Pharmacy Board of Australia Registration Standards, the Pharmacy Board of Australia Codes and Guidelines including the Pharmacy Code of Conduct for Registered Health Practitioner and WACHS policies and guidelines.
