Interested in working in a team that; embraces all things recovery and holds hope for people with severe and persistent mental illness (SPMI) that are at risk of falling through service gaps? Someone who is interested in supporting culture change, capacity building and service improvement for consumers who have complex and high care support needs accessing SWSLHD MH services? If you answered yes to the above, then the Pathways to Community Living Initiative (PCLI) team is looking for you. About the Opportunity The role involves working alongside MH teams to support consumers transition into their sustainable and safe home in the community. You will also work alongside community teams to support consumers with SPMI who are at high risk of a long stay in hospital if their community circumstances are not sustained. The work aims to improve outcomes for consumers who are highly vulnerable to falling through service gaps.
The position is part of the SWSLHD PCLI/IDMH and NDIS team. It works alongside other SWSLHD MH services (inpatient and community) and the wider PCLI state network.
Professional qualities desired are: staff comfortable working in challenging environments whilst sustaining enthusiasm for individual and systemic advocacy. Staff able to recognise and celebrate the small wins that are often overlooked due to the complexities and challenges faced.
The role is full time however those interested in part time/job sharing are encouraged to apply as this can be negotiated. The PCLI team is a district wide service however the team hub is based in Liverpool hospital.
What You'll be Doing The Pathways to Community Living Initiative (PCLI) senior clinician will provide leadership, mentorship and supervision to ensure the delivery of evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team. The clinician will carry a PCLI clinical workload, appropriate to the needs of the service. The Senior Clinician will work across multiple services and sites with clinical teams to support the assessment and progression of safe transitions of adults consumers with severe and persistent mental illness (SPMI) into the community. The clinician will also work with PCLI team and alongside other MH community services to improve outcomes for those at risk of an admission due to th eir complexity and potential breakdown of supports.

Relevant qualifications as an Occupational Therapist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW) with recent extensive experience working in mental health. Demonstrated extensive experience in mental health rehabilitation providing advanced evidence based, strengths focused, recovery orientated assessment and treatment for consumers with severe and persistent mental illness (SPMI) and complex needs. Demonstrated skills in leadership and clinical supervision, within a multi-disciplinary treatment setting. Demonstrated effective interpersonal skills (including excellent negotiation and advocacy skills), problem solving skills and personal resilience when working with a range of stakeholders. Highly developed written and verbal communication skills and competence in the use of information technology systems. Demonstrated experience in the planning, implementation and evaluation of local service delivery using the quality improvement process. Ability to work independently and in consultation with the multidisciplinary team. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
