Position Profile: As part of a multidisciplinary team this role provides advanced recovery-based care to consumers of the Community Mental Health Service and functions as a Care Coordinator undertaking clinical assessment of an allocated case load of consumers with a mental health concern within the Active Recovery Team. The Senior Mental Health Practitioner will contribute to the development and monitoring of the specialised team, initiate quality improvement practices and implement changes and improvement for the mental health service. The Senior Mental Health Practitioner provides clinical support and supervision to the Active Recovery clinical team members; support and mentoring to team members from partner service providers.
