Physiotherapy, OT & Rehabilitation (Healthcare & Medical) Government - State (Government & Defence) The Community Rapid Response Service (ComRRS) is an innovative service model that provides high acuity/high frequency intermediate care for patients with acute illness/injury or acute exacerbation of chronic and complex conditions, with the aim of avoiding hospitalisation. The ComRRS model is based on a "shared care" relationship with the patients GP, and a highly collaborative approach to care need for the referred clients is essential. The team are highly skilled clinicians led by a nurse practitioner and have a strong focus on excellence, responsiveness, and a holistic client-centred approach. The service receives an average of 100 referrals a month and is very highly regarded. Position Features: Some travel within the region will be required The Community Physiotherapy team within Primary Health North. Our team works across all the Primary Health sites in Northern Tasmania. The Community Rapid Response Physiotherapist will work predominantly in the Launceston area, and be based in the Allambi building, Howick Street, Launceston.  Being able to deliver a highly effective and rewarding physiotherapy service that aims to support hospital avoidance strategies and early intervention. Being part of a highly collaborative multidisciplinary team that has a focus on best practice. Greatly varied case mix with autonomy Nature at your doorstep that enables you to explore Tasmania's most unique environment. The role: Within a Departmental Framework, and in accordance with Agency policy and procedures and professional code of conduct, the focus of the Senior Community Physiotherapist is to: Be responsible for the efficient and effective provision of physiotherapy services to clients in the service location. In consultation with the Team Leader - Community Physiotherapy, maintain physiotherapy services in Primary Health sites including supervision and support of staff and students. Contribute to health promotion within the community. Participate in relevant quality improvement activities to ensure safe and effective delivery of client care. Ensure that client care is provided collaboratively with other members of the multidisciplinary team across relevant areas of the northern region of Tasmania as required. Primary Health North Community Physiotherapy Team: Being integral to the Community Rapid Response Team and implementing hospital avoidance strategies.  Working within a larger multidisciplinary team, but autonomy in the role on site Working at a variety of Primary Health North sites with challenging caseloads with good variety. Salary:  $106,966 to $123,013 per annum. Our Employer 11% superannuation contribution is on top of this amount.
