About the roleWe are seeking a values-driven and proactive Senior Case Worker! You will provide support to the team ensuring services are provided with the Model of Care.
Additionally, you will support day to day service delivery and ensure that case workers provide case management, assessment and referral, education and support.
This is a permanent full-time position based in Dooralong, NSW.
Salary and conditions is accordance with the Social Community Home Care and Disability Services Award Level 5.
You will successfullyProvide efficient day to day service delivery through education, interventions, group work, assessment, referral, and case work within a recovery-orientated and person-centred environmentDevelop and monitor individual treatment plans and maintain case management documentation, records, reports, and data collection, utilising established case planning proceduresDemonstrate commitment to an environment that values continuous improvement and innovation in the delivery of high-quality servicesAcquire and maintain a good working knowledge of referral and support options for clientsReflective practices are implemented for the team and contribute to improved outcomes for clientsParticipate in promotion of the service via conducting presentations to local servicesStrong referral pathways are built and maintained with internal and external stakeholders, resulting in beneficial outcomes for participantsYou will haveMinimum diploma level qualification in AOD, community services or related field ORDemonstrated high level of sector knowledge and skills attained through previous appointments, service, and study/developmentRelevant experience in supervising staff preferredA current NSW Driver's LicenceRelevant state Working with Children Check is requiredWhat we offerAs a registered NFP we offer our eligible employees real and meaningful benefits such as;
