Be part of a team, providing specialist and intensive assistance to help people in the care finder target population to understand and access aged care and connect with other relevant supports in the community. Work closely with the Team Leader in delivering assertive outreach to clients, service providers and the community across multiple LGA's. Undertake holistic assessments and support planning to determine and document individual needs. Commit to great customer care and an innovative approach to effectively engage with people in the care finder target populations. Joining us, you'll join a team of dedicated individuals who are focused on creating positive social change, together and your work will be diverse and impactful. You will be part of a team implementing the care finder initiative  across Dubbo, Narromine, Gilgandra, Warrumbungle and Mid-Western regions.
We have two positions: one permanent part-time (30 hours per week) and one fixed-term full-time till 30 June 2025. Travel is a component of this role. The successful candidate will have:
Relevant Tertiary Qualifications e.g. Community Services, Human Services, Social Work or related discipline and/or equivalent experience. Working knowledge and demonstrated understanding of aged care supports and services and other relevant supports that are available in the local community. Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationships. Experience working in diverse environments including office, health, community services and home settings.
