Responsibilities: Work under the directive of TORANA CARE and Clinical Coordinator (CC) as required.
Facilitate training of junior support workers for clients needing high intensity and nursing care.
Provide professional guidance to Assistant Nurse colleagues.
Provide constructive feedback to TORANA CARE and CC as required.
Assist with fortnightly Care Plan Management and Collection.
Implement and assist CC in the review of individual participant care plans.
Assist with wound assessments and dressings as directed by CC.
Assist with medication administration as per Webster Packs and medication plans.
Assist with specialized care: Tracheostomy, PEG/PEJ, catheter care, stoma care.
Assist with clinical assessments as per CC/RN directive and written instruction.
Contribute to team morale and team culture in a positive way.
Assist with new employee onboarding and skill up-lining.
Maintain WWVP, First Aid/CPR, annual influenza and mandatory training.
Continue ongoing professional development.
Assist clients in all aspects of everyday living within scope of practice.
Ensure best practice takes place in all aspects of participant daily living.
Utilize hoister lifter as required (with 2 people).
Assist with participant personal care/daily living needs.
Assist with maintaining appropriate infection control and WHS measures.
Assist with domestic tasks as required.
Assist participants with community access and transport services.
Provide meaningful social interaction with participants.
Advocate for participants, relaying any concerns.
Maintain clear and factual documentation.
Report directly on shift to RN and CC for SIL Property.
Report directly to CC &/or TORANA CARE CEO for Community.
Understand Mandatory Reporting and legal responsibilities.
Attend training/education sessions and in-house team meetings.
Attend general team meetings.
Required Skills: Extensive Disability, Aged Care, Specialised Experience.
Possesses Specialised Skill Set(s): Diabetes, Tracheostomy, PEG/PEJ, Stoma, Catheter.
Required for all rotating shifts: mornings, evenings, and night shifts.
