Hammond Care Horsley is currently looking for a Permanent Full-time Manager to support our complex behaviour cottages supporting people living with dementia to join our team.
With our unique model of care which places the residents needs as your number one priority, you will help create a home away from home for our residents.
This role requires you to provide tailored care including personal support within the residential home.
About the role: As the Manager for complex behaviour cottages supporting people living with dementia, you will be a core member of the Horsley management team and work with a wide variety of people including the Residential Care Operations team, Local Health District, SDAs (RNs), SDCs (Carer's), residents, resident representatives, and the multi-disciplinary health team to ensure resident outcomes are met.
To be successful in this role, you will: Advocate for residents to ensure their care is person-centred and focused on their strengths, capacities and promotes their well-being at every stage.
Facilitation of clinical care delivery by the SDAs (Registered Nurses) and SDCs (Carers).
Hands-on modelling, coaching and mentoring in clinical and complex behaviour management areas and processes Consultative in case management and clinical decision making Clinical oversight and professional competence as a Registered Nurse with experience in complex dementia-specific care Support and investigate clinical/resident incidents as required Communicate effectively with all stakeholders, both within Hammond Care and external partners to see the best outcomes for the residents.
To be successful in this role, you will have: Tertiary qualifications in specialised dementia and/or mental health service nursing for older people.
Significant experience operating as a consultant or specialist nurse preferably within Aged Care Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff.
