The heart of the role:As our Care Manager, you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier, and happier lives.
The Roving Care Manager role will be offered as a permanent full-time position to oversee and support the clinical operations of a few Homes in the Northern Rivers region. Duties include:Oversee the daily clinical operations of the care home.Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.Review clinical incidents, complaints, and changes.Lead compliance with AN ACC assessments.Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.Prepare and conduct clinical governance audits.Oversee medication management in accordance with resident needs and legislative requirements.Liaise with allied health, medical practitioners, and other services to provide optimal resident care.The BenefitsVIVA Happier & Healthier - our employee wellbeing program - live healthier and happier.5-x free telehealth GP appointments through Blua (our digital health platform) for you or your immediate family.Career development; You'll have a real say in shaping the future direction of the business and your own career.Access to leading training and professional development - supported by our quality education team and expert clinicians.Comprehensive induction and buddy program - we set you up for success from the outset.Be recognised and rewarded - Bupa Love Reward & Recognition Program ensures that the gratitude of our residents and their families is passed on to staff who deserve it.What will you bring?Bachelor of Nursing with current AHPRA is essential.Post grad qualification in aged care highly regarded.Proven track record in delivering quality clinical care within an aged care or acute setting.Excellent clinical assessment skills and care planning capabilities.Highly effective communication, leadership, and relationship management skills.An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.Be at the Heart of It – your new team is ready to welcome you!
