About The Role Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management.
Clinical delivery and supervision of non-clinical team members in a community setting.
Intake processing, clinical care planning and reporting.
Conducting client care assessments with a focus on Consumer Directed Care.
Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding according to the client's budget.
General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation.
On-call roster.
Qualifications Qualified Registered Nurse.
Current AHPRA registered.
First Aid & CPR.
Australian National Police check.
Professional Indemnity Insurance.
Experienced in assessing and planning care needs within the community.
Valid Driver's Licence and reliable presentable car.
A smart mobile phone (iPhone / Android).
Preferred Skills A passion for high quality client-focused care in a community setting.
Cert IV in Training and Assessment - highly desirable.
Exceptional communication skills.
Able to work autonomously or within a team environment.
Excellent time management and organisation skills.
Commitment to ongoing professional development.
