Right at Home Sunshine Coast provides high-quality in-home care and assistance to our valued clients in the Sunshine Coast, Noosa, and Gympie regions. We support people living with Alzheimer's, dementia and cognitive decline, disabilities, post-operative care needs, and older Australians. Our services help clients remain safe and independent in their own homes. Our mission is to improve the quality of life for those we serve.
About the role

The Care Coordinator will be responsible for:
Building strong connections with clients and their families Assigning staff to clients based on staff qualifications and availability, as well as client preferences Organising support for clients with disabilities and monitoring and reporting on their progress Referring clients to agencies that can provide additional help Assessing client needs and resources for health, welfare, and other facilities and services Responding promptly to emails, enquiries, and requests from clients, staff, and management Maintaining accurate records of incidents and feedback as required Reorganising work priorities to meet the needs of a fast-paced environment Supporting all clients to meet their goals Conducting Client Care Assessments and Care Planning Providing clinical guidance to other team members for client management Discussing and promoting Right at Home's services with medical professionals and referral sources Minimum Requirements:
Current AHPRA registration as a Registered Nurse Experience in assessing and planning care needs Understanding of the Aged Care and Disability systems A strong client focus based on their goals, preferences, and needs Current National Police Check (or commitment to obtain prior to start) NSW Working with Children Check (or commitment to obtain prior to start) NDIS Worker Screening Check (or commitment to obtain prior to start) NSW Full Drivers Licence All candidates must have the right to work in Australia Organisational, administrative, and time management skills #J-18808-Ljbffr
